Social Prescribing Self Referral Service

We’re pleased to announce that the Mary Francis Trust Social Prescribing Service is now accepting self-referrals from anyone (18+) registered with a Banstead GP surgery. 

What is Social Prescribing?

Social Prescribing provides support around emotional wellbeing, social isolation and loneliness, finding employment or volunteering, building and maintaining a healthy lifestyle, welfare advice, or carers support. It offers a non-clinical signposting and bridging service to find support in the community that is right for you.

How to self-refer:

There are a few ways to self-refer:

1. Download the referral form from our website and send it back to us: Find the Social Prescribing page here: Social Prescribing - Banstead - Mary Frances Trust
2. Email socialprescribing@maryfrancestrust.org.uk and ask for an electronic referral form to be sent to you.
3. Call 01372 375 400 and ask for a referral form to be posted to you.

 If you have any questions about the service, please contact the Mary Francis Trust on the number above or speak to your GP Surgery. 

We also still accept referrals from GPs and professionals.
